The Recover Support Navigator is responsible for Case Management,
Network Support Facilitation, and Case Coordination.
Salary: $20.00/hour

Duties & Responsibilities
- Screen clients to determine eligibility for SOR Grant.
- Meet with clients and create a wellness plan.
- Develop set of goals and objectives identified by client.
- Implement goals and objectives in integrated service plan.
- Deliver services to clients in the community (home, treatment
facility, diversionary unit).
- Obtain insurance authorizations and conduct concurrent reviews.
- Assist client with transportation needs; provide transportation.
- Arrange referrals for services when needed such as primary and
behavioral healthcare.
- Coordinate care with inpatient staff, aftercare facilities and
community partners.
- Provide linguistically appropriate and culturally sensitive case
management services.
- Document all services in EHR.
- Participate in SSTAR MAT meetings and SOR program meetings by BSAS.
- Participate in training as it relates to opioid use disorder,
stimulant use disorder, MAT, etc.
- Provide meaningful links to treatment, recovery support services,
and mutual aid.
- Form therapeutic relations with individuals affected by SUD.
- Provide intensive care management upon admission and keep in close
contact until the individual reaches’ stability.
- Provide patient education on community resources and programs.
- Provide target interventions to priority populations.
